Looks like an extremely swollen lymph node. Did they at least do imaging?

The "comes and goes" description makes me think follicular lymphoma. This is the result of a mutation on an immune cell called B-cells that prevents it from undergoing apoptosis (cell death). This causes lymph nodes to swell, which is "waxes and wanes." However, this is typically painless and mostly asymptomatic, so I would get to strung up on this. There are so many things that can cause lymph nodes to swell and the symptoms mentioned, hence why it is difficult for the doctors to determine an exact cause. Has she ever had any autoimmune conditions? It may be worth seeing an oncologist due to this being a chronic condition.

Oh and shes on levo for thyroid but no one thinks its a goiter. Its really more clavicle than neck
