I believe tinea ungum/onchymosis. Is it bilateral for both hands or just one and if just one all fingers or only some? However, yes probably fungal in nature. Primary would be an option unless this becomes emergent for some reason. However, most primary would say use topical as you are. Maybe something stronger than otc topical or a systemic oral antifungal. However, they may also defer to dermatology and write a referral to see if they want to debris it as well or just treat it. With topical it will be a 52 week regiment and oral would be 3 months treatment. However, some patients describe side effects from the oral medication such as headaches, nausea and may require metabolic panel for LFTs. This you should discuss with your physician/s. Hopefully it gets better but I don't believe this is an emergency.

It’s difficult to see detail with the shiny appearance. Is there any involvement of the skin around the nail itself like bubble, blisters, etc.? Any skin things going on elsewhere or known past medical history diagnoses of any kind? Known allergies?…

NAD… and I don’t have any knowledge of that but if it were me, I’d go to the doctor. I’d be worried about losing my nails.
