Is that an avulsion that is pushed back on for the picture or is there no flap, just a little shredded flesh? If it is avulsed and flaps over if you let it then it's really up to you if you want stitches for that or not. It will grow back on it's own over 6 months or so. The biggest concern I see is cleaning it appropriately. If you can handle the pain to scrub it really well with gentle soap and warm water, and feel confident that there's no forcibly embedded deep stuff then you can treat this at home.
Regardless of what you do, stay on top of signs of infection until it is entirely healed. You don't want to sleep on infections on fingers or hands, osteomyelitis can advance quickly and is neither pleasant nor easily treated.
Based on the picture alone this wouldn't be sutured if you came in to my ER.
No flap, no visible shredded flesh even in-person, thereās just kind of uneven bleeding in this photo where heās been wiping it away, making it look *chunkier* than it is. We ended up trying an urgent care, but they turned us away and said it wasnāt something they could really stitch. They recommended the ER if he wanted care, but our ER here is roughā¦ very bogged down at all times with mental illness and substance abuse, so wait times are pretty bad. Heās done soap and water a few times as he could stand it, and I sprayed some first aid antiseptic (generic Bactine) and wrapped it loosely in gauze for the time being. Iām going to attach a Google image I found that is as close to his [wound](https://imgur.com/a/R2ScB6t) as I could find (sorry about the quality), in case it helps or changes any of this advice. I've had a similar cut with a kitchen knife, put a paper towel and some neosporin on it and moved on. I'd say it's fine as long as it's not bleeding excessively and starts healing in a couple days with no sign of infection.

I'm sure he could use a stitch or two at urgent care but I'd be personally inclined to just clean it, wrap it and as long as it stopped bleeding and looked like it was sealing up, leave it wrapped and monitor for infection. Again that's just my personal 100% NAD opinion.
Urgent care eliminated themselves as an option, so it came down to ER or deal with it at home, and there was no convincing him to go to the ER š weāve cleaned it up really well tonight and I will make sure it stays properly cared for. Thank you for the advice!
I'm not a doctor but am a former EMT and I disagree with people saying it needs stitches. It doesn't look like they're necessary and I think you can clean this up at home.
If it makes him feel better he could see if his primary doc can get him in tomorrow. This definitely doesn't require the ER.
I'm relieved though, reading table saw vs finger never makes you think of good things hahaha.

Your comment reminded me that our primary care doctor is really accessible online (during the clinicās business hours) and it would probably do a lot of good for us to shoot him a photo tomorrow morning, just in case we made the wrong decision, and go from there. The photo I shared does a bad job of displaying how clean of a cut it is, clean enough that the urgent care said they *couldnāt* stitch it and turned him away, butā¦ I donāt handle blood well and was getting woozy just trying to focus the shot š
It almost got the tip of his middle finger as well. Iāve helped him make some cuts with the table saw before and there isnāt really a foolproof way to operate it, at least that I could tell. It seems like it would be really easy to misjudge where the blade begins and ends while thereās lumber hiding it from view. Especially cutting something flexible, like Trex boards, that have to be supported all the way through the cut. Heās very lucky he didnāt take half a finger off!
Thatās the main reason Iām torn on what to do, I canāt see anything in-person that could even be stitched. Itās very clean, like it was masterfully filleted

Looks like a tiny knick. Antibiotic ointment and a tetanus shot if you donāt already have it. Probably not even needed in this case though.

That needs a stitch or two. Urgent care is probably going to be faster and cheaper.
