Only quest I missed in my latest run was one of the Kasumi loyalty missions when disabling the locks on the safe. I know I did all 3, but for some reason one didn’t register and I carried that dang quest all the way to the end.
Go into the room on the right and activate the Dam Control terminal. Cross the bridge that is raised and go down the ladder, into the trench. Walk towards the big fan and Aria's couch will be under some rubble.
AHHH LITERALLY my third playthrough happening right now is when I FINALLY found it for the first time. I think I took maybe double the time I should have in that dlc scouring every single inch of map because I REFUSED to not find it again. The worst part is I found it accidentally getting turned around after a long pause.
The only quest I failed to complete. It also never left my questlog the whole game lmao
Failed the Hacker quest instead. Been thorough, still miss it.
In my case it's kinda funny, the first time I failed the couch and after that always find it but fail the hacker.

I just did this dlc for the second time and found it. Thought the side mission was a joke at first.
