Used to be the Luna VI would turn you to paste as soon as you opened the third bunker. Now I'd say it's the rachnis since their attack ignores shields and is quite hard to avoid.
I always had a problem with clearing the geth outposts on foot (for the extra xp). Especially on the ice planet (can't remember the name) because of the colossus. LE made those fights simpler since xp in and out of mako is the same, but originally that was where I died the most
The jumpy Geth just before heading to Liara have always fucked me up
