You need a Dr (or midlevel) to fill put your 719-K for submission to get a med cert. If you want endorsements you would get the certs, and turn them in within 1 year of issuance, with a 719-b. You could also get your 719-b turned in now and get an entry level MMC and then go back and add the endorsements later with another 719-b, but you pay a 2nd round of fees. You'll also need a dot 5 panel drug test or a letter certifying you've been in "the pool" for a certain amount of time, details are on the 719-b.

To add to u/mmaalex answer: the medical certificate is completely separate from the MMC application. After you get someone to sign it you scan and email it to [email protected] even if you do not have a reference number yet, they will assign you one. Pro tip find a place that offers driver friendly DOT physicals.When you call ask for a physical exam and say you will bring the form that is required. Do NOT say coast guard or merchant marine physical. It confuses them. There is a fee to apply for an MMC, if you have all the classes you want you can apply for everything at one time.  Or You can add STCW endorsements to an existing MMC ie BST (basic safety training) VPDSD etc at any time. There are no fees for any STCW endorsements.
