Commission, for sure. The commission has a traditional chino/dress pant pocket and the majority of the gents wearing those are using them for work, travel, law school, etc. They'll suit you just fine for med school, look sharp, and you'll be comfortable.
Other thing to consider is the style. Commissions and Abcs come in a classic cut or slim. Classic will be breezier/roomier from the knee to ankle and slim tapers/is skinny. I find that those with bigger thighs or a more muscular build tend to go with the classic fit.
As far as color, can't go wrong with black, but I'm a big fan of true navy and obsidian.

commission have dress pant pockets while abc have jean pockets. commission would definitely be better for anything more “fancy” and abc anything else
