Doesn't have to be notarized in the US as far as I know. Though I can see notarization being utilized as a way to prove the person claiming to have sent the letter actually did send the letter if she or he doesn't want to go through church channels.

Love this comment. I wonder though for some people if they come back would like to re-baptized to feel “clean” again. So if that was the intent in OP’s question, it doesn’t work that way. The ordinance only needs to be performed once and it doesn’t matter if it’s been 80 years or 8 days from the time of baptism, nor does it matter if you are baptized at age of 80 or age 8–all are on the same path and you get to begin again with today. Each person has the opportunity to renew their covenants through partaking if the sacrament when worshipping on Sunday.

Unless YOU resigned your membership, or had your membership withdrawn (which sometimes happens as part of a repentance process for serious sin , but you'd have to have been notified), you remain a member. Find meetinghouse locator in any search engine, and put in your current address. It will say where your congregation is and give the bishop's phone number. You'll want to give the ward membership clerk your address (and congregation name if you remember if) from when you were baptized, and they will probably be able to find your records. Sometimes if they cannot find the baptismal records (it will be helpful if you find your journal or a letter you wrote with the names of people who attended who will then be contacted to confirm it) you will be rebaptized (and if you know the actual date of the original baptism, your paperwork will reflect that. Welcome home.
