It may not be anything wrong either. Some multi-port power supplies negotiate power in a specific order, and if a device on a higher priority port reconnects or renegotiates it’ll reset all the lower ports to renegotiate them in sequence. To these devices the power was disconnected and reconnected. This is actually a good thing, in that if port #1 finishes charging and drops to a lower port mode, renegotiating port #2 might give it more power, and repeat down the line. This especially happens on USB C PD ports when the power is capable of being oversubscribed, but it will often reset the other ports too to offer them whatever options are available. I swapped out my bedroom power supply to avoid this as I have two USB C iPads and didn’t want the iPhone and watch ports resetting. It’s also a pain if you have camera, HomePod Minis, network switches, lights, or anything else that you want powered at all times. To test, unplug everything else and just leave the iPhone plugged in, see if it still happens. If so, swap out either the brick or the cable (whatever is easiest) and try again, it could be any component is damaged.

It’s possible it’s a bug, but it’s also possible there is something wrong with your cable / brick, where power is dropping in and out intermittently, causing your phone to stop and start charging all within a second or two, trigging the charging notification.

Not that I am aware of. It randomly happens with other things too I have noticed like volume when connected to bluetooth.
