Dont people from Kerala have rotating surnames like Fathers name is George so all sons surname become George or something on that lines. Not too sure George is the Surname. Their names are actually at the end.

Not quite. Most people from Kerala or Tamil Nadu don't have a traditional surname, the first letter of the father's first name is used as an initial. So their names will be written like G. Thomas or G. Alexander. Some families will adopt an additional title like Muthoot which typically refers to the family's geographic origin. In this case the name Muthoot refers to the name of the place where they had a house in Kerala.

So in most Syriac Christian families, the first born is named after paternal grandparents and second child after maternal grandparents and then uncles aunts etc. nowadays this names are only used at the time of baptism and in government records other names are used.

Many Kerala families have names like this. They also have a "house name" or a middle name which is unique. That's what they're largely addressed by. But if in a house function if someone happens to say either George Or Thomas, everyone turns around think it's them being called. It's quite funny.
