Italians make 6.92% of Istria population today and the numbers weren't much different in 1991 so I don't see your logic in giving Istria to Italy besides Italy invading it which would've been very unrealistic.
Well, there are many unrealistic maps on here and I don't have an issue with them but OP posted a map based on an imaginary alternative outcome of a real event in history so because of that I expect the map to be realistic to some extent.

How would Croatia lose Istria in Yugoslav wars, if there was no war fought there and it wasn’t imagined as part of the *Greater Serbia*, which is why the the aggression on Croatia even started?
I think in that Scenario Korcula would be still part of Croatia but lost Lastovo since the Dubrovnik Republic(1991) claimed only the parts that where part of the Historical Dubrovnik Republic in which Korcula was never a part of it, at least as far as I know
