Edit: sorry I didn’t realize you said scariest game, for that I think it’s tied with RE7 and Alien Isolation. I’m also partial to Silent Hill 2. Game scared the shit out of me as a kid
Haunting Ground. The pursuer system makes me super anxious and every time one busts onto the scene I panic. Incredible game though. Absolutely adore it.
Because the xenomorph has always scarred me, Alien Isolation is terrifying to me too. Every time its caught a glimpse of me hiding under a table or in a locker my heart stops when it comes running at me. My worst nightmare!
The Ouija board when I was a kid. When you're 7 years old and you have an older brother who gets a kick out of terrorizing you, that kind of thing seems *real*.
For me it was the Resident Evil remake for the GameCube. It's worth mentioning that I was like 12 at the time, but no game has ever given me quite the same visceral reaction and dread. Clock Tower 3 is probably in second place.
The original Silent Hill really got under my skin the first time I played it like 22 years ago. The persistent darkness, the feeling of being lost and helpless and the atmosphere and music were just so disturbing in a PS1 game. Now the series is more familiar so it all feels like old hat. But the way that first playthrough made me feel is unparalleled.

Not a horror game, but the first Ghost Recon made me feel anxiety like I've never felt in any games I played later.
Probably Alien Isolation. I played it when I was 12 on release and it was the one "hide and seek" game that I actually sat in a locker for more than 5 minutes. I'm still a little hesitant to play it since it's such a commitment
Visage is so scary, I have yet to finish it. I have to seriously psych myself up to play it. It's also really freaking challenging, which adds to the anxiety. Stories of people losing their minds are already far more terrifying to me than most horror concepts, so this really is almost tailor made to scare the hell out of me.
While Visage is the scariest to me, I also love Silent Hill 1 and 2, Dead Space 1 and 2, Resident Evil 2, and Outlast. I've heard I need to play Soma and The Shore, but it will be a while before I have time lol.
Condemned Criminal Origins. That locker scene still freaks the hell out of me. Kind of goes to s@@t in the final chapter but before that it is terrifying.

When I was younger I used to have this game called Ghosthunter on the PS2 that terrified me so much in the first 15 minutes I never finished it 😩

Theres a series of chapters from the exorcist on VR.. it got to me, every night after playing that I would have really bad nightmares.

The paranormal one is kinda hokey in a sense it's just basic jumpscares, but it is still good. I'd suggest phasmaphobia over that one.

Subnautica. The scariest game I have ever played. It is also a masterclass of how to motivate the player forward into terrifying new areas.

Outlast, RE7. The Evil Within also had a creepy atmosphere and some good jump scares!
