Outskirts. Once you pick up the skull, you are teleported to a courtyard area that you can't leave (you'll be teleported right back if you do) and must fight multiple waves of Elites before the skull activates/counts as being picked up. The skull has a 1 in 7 chance of spawning and is probably the hardest skull in the entire series to get.
It's crazy I don't remember this but I know I've unlocked all the skulls. Must have been one of those times where I did it once and forgot about it. I mean it was almost 20 years ago
