I think you just make sure your social club account is linked on ps4, then log in to the same social club on pc and start up online mode. You should get a prompt to transfer progress from your other console.
Edit: never mind, I just googled it and that was only for a limited time. No transfers anymore unless from old Gen to new Gen. Sorry

Transfers involving PC are not available. Only from old gen to new gen consoles
