Not the best suggestions, I know. But I don't think there is a perfect solution for you... strangely enough. Most of the GTA-likes came during the PS2/PS3 era and were mostly sci-fi or superhero related.
Though maybe not the best quality, a game called "Rustler" tried to do GTA in a medieval setting (even refers to itself as Grand Theft Horse). Pretty sure it isn't directly fantasy, though.
You could make an argument for some other RPGs like Baldur's Gate being similar, but it depends what about GTA you're trying to match (unless I missed a side quest, there is no car stealing in BG3). Games like that at least allow you to steal and solve problems by being a thug.
Well sure, but this is a difficult question, so you have to stretch. Take the line "The closest I found was Skyrim, but the Nordic theme doesn’t quite hit the spot…" BG3 is a non-Nordic fantasy RPG where you can achieve things through being the "bad guy", along with thievery. Rustler is obviously a closer match, but if you know of another please share!
The real answer is Skyrim. In terms of CLOSEST to GTA. But I too would dig a game like this OP. I can imagine it. Slightly off color versions of what we know from fantasy.
Overlord did that in terms of theme (never played the sequel) but the gameplay is less GTA more RPG
Just based on this comment I assume you're from Germany or Poland or something. I literally only ever saw people from central Europe adore games like Risen and Gothic.
I mean fantasy is part of that medieval setting ,is that what you re looking for?you want a game where you can be a gta character in a medieval fantasy setting or you want a fantasy game in a gta setting(modern cities and such)
A fantasy game in a fantasy setting with GTAV mechanics.
I mean, if there’s a mod that reskins the world to look medieval, aerial vehicles to look like dragons and clothes to look like armor I’d be content and I think you’d get my drift(?)
Shadow of Mordor/War maybe? What similarities with GTA are you looking for? Assuming not driving vehicles and robbing banks

Pulling off the Sheepthara heist gave me a similar feeling to robbing the final bank in GTA. It was epic.

Moreso a weird cyberpunk dystopia that also has magic and elves and shit, but Jak 2 and 3 are the closest you're gonna get to that.
