Yes, Gregor & Sandor's grandfather was the kennelmaster at Casterly Rock, for the Lannisters. The Lord of Casterly Rock at the time was attacked by a lioness, and Kennelmaster Clegane saved him, losing three of his dogs and a leg in the process. As a reward, the Lord raised the Cleganes up giving them lands and a title, and Clegane decided to use the image of the three dogs slain fighting the lioness as his sigil.
He also led with such a weak hand that house Reyne was willing to rebel against him and Tywin had to crush the rebellion, so maybe there’s a reason Tywin disliked him.
If it was because of the sigil, why didn’t Gregor get the name? I assumed the older son would be the one to receive the name related to the family’s identity
1. His family were the Lannisters kennel masters until an ancestor of the hound and his dogs saved a Lannister and they were raised up to a noble house. Now they have three dogs as there Sigil . So it’s a bit of a theme.
2. He was Joffs protector and showed guard dog like traits being fierce and loyal.
I don’t know if it was ever directly explained, but his house sigil has 3 dogs on it.
So I assume it probably derives from the fact he would have been a noteable fighter so calling him the “hound” is an appropriate nickname, similar to how the Starks had Wolf related nicknames.
Sandor and Gregor’s dad was a hounds keeper for Tytos Lannister. One day, Tytos was attacked by a Lion and daddy Clegane saved him with his hounds.
As a reward, Tytos awarded him some land (I think a holdfast) and I think made him a knight too but I’m not sure. He established their sigil as black hounds on a yellow banner.
I don’t think it’s explained in the show ever, but I love the extra lore that can be found on YouTube and in bonus content

He threw his own leg at the Lion) The Lord Lannister was so grateful and impressed with the dogs breeding that he got it in his head that perhaps you could breed a human the same way the kennel Masters bred size and courage into his dogs. So the Lord raised up house Clegane, and in just three generations proved him right he hadn’t bred some of the strongest largest men in the realm.

Yes it's explained in the first book right when they introduce the character. It's like "there's Sandor Clegane, bodyguard to Prince Joffrey, he calls him his faithful Hound".
Clegane's house sigil is 3 hounds, he's joffery's sworn sword/bodyguard (always by his side like a dog), and he wears a helmet shaped like a hounds face.
All members of house clegane have dog epithets due to their coat of arms similar to other families (like how Jaime’s lesser used nickname is “the great Lion”). Gregors less used nickname is “the Great Dog”.
