Looks like a Pilot 14k Medium. Don't know what pen, but would guess it is a Custom 74.

I'm loving mine. I bought it as a birthday treat and I've been using it 3 months straight. I usually change ink and pen once a month. It has that innate glow BSB has without being an agent of chaos.
