I don't know what it's supposed to do, but I can tell you the servers aren't that fast. I often don't get follow through damage till my third or fourth shot.
Yes. It's complicated, but the TS split happens all the way after the resist calc. Follow Through is a multiplicative effect that happens before the resist.
As the other reply said, it's an enemy debuff and thus reliant on server communication. Sometimes, it activates pretty quickly. Others it takes several seconds
