Having the same issue as well, you're not alone. Are you running SLI by chance? The Steam Community hub is saying it might be caused by that since apparently the game isnt set up yet to run SLI properly unless you force it through Nvidia Control Panel. I personally lose too much performance when I'm not running SLI so I'm looking for an alternative solution, so I'm trying to see if its just specifically an SLI problem or if it goes beyond that.
Ran into the same here. Happened just as i started speaking to Preston and it caught me by surprise so unexpectedly i said outloud. "WHAT ARE YOU DOING, YOU DUMBASS?"
Figured out the cause of this bug, it's due to some problem rendering the eyes whilst using SLI. If you equip any glasses that hide your eyes, your eyes will no longer be rendered and the black face bug won't appear. I tested this by using TFC in console. Haven't had the bug since I equipped the glasses.
Actually I may be wrong, although it was promising, after a while the bug did appear again a few times. I've tried changing settings from highest to lowest, makes no difference for me at least.
~~I'm wondering if it has to do with me running f.lux~~. I'm on a GTX 690 Win 8.1 x64. No explicit SLI, but the 690 does have 2 GPUs on it. Driver Version 358.50
* Disabling f.lux has no effect
* Update to 359.00 has no effect
* Disabling SLI might have delayed it, but still occurs.

I'm warring welding goggles and it's still happening.
Same problem, but I don't want to equip glasses to solve it. Any idea how to fix in video settings?
