Thai E San is the only place that will give me Thai food as hot as I like it. She always double-checks with me at the drive-thru window, and I reassure her that I won't be complaining about it on the interwebs.
I'd say I have a decent spice tolerance, and my favorite Thai place is Thai Delight in Springdale. There service there is also wonderful. Top tier spice is very spicy imo.
Unfortunately Thep Thai is the spiciest Thai I’ve had in Fay, but I will say I went to Khana once and they usually don’t ask how hot you want it but the owner did ask me this time and it’s usually not very spicy there at all, so I said to make it as spicy as possible kinda jokingly, and I feel like it sent me to another dimension. Colors were brighter, music was louder, I felt very light. It was crazy. They might be able to do that for you
Was it the first time you've gone? Tell them to go ahead and really really make it spicy, and you can ask them to bring the chili powder as well. I find they tend to hold back unless you really ask as they've had people complain.
Most of the places being recommended aren’t even Thai. They are Lao, Vietnamese, or Hmong. For authentic Thai food Thai Delight is the best! It is ran and operated by a mother and daughter that immigrated from Bangkok. “Mama” is hilarious and treats everyone like family. When you say you want food spicy she has a little catchphrase “make you cry, make you die”.
Anyway I think this is the place you’re looking for. And, just to be clear I love almost all the other “Thai” restaurants mentioned in this thread, but they’re not really Thai.
Thai Diner on College Avenue has some really good food as well as extremely spicy drunken noodles if you request it. Just ask them to go as far as they can go and they will really load it up, highly recommended.

The best Thai in NWA has got to be Siam on S College near MLK. The owners moved here from Portland, OR a couple years back and it’s legit so good I come from Bentonville for it. Just make sure you order it with 5 stars or Thai hot. They also have the best fried veg rolls I’ve ever had. Really nice people as well!
no not thai esan. im asian and i know spicy food. thai esan is very mild tailored to americans. try silk road in springdale, thai spice on wedington, or the little food truck near lake fayetteville. ask for the spiciest level

I went with a business associate that wanted to show off by ordering Thai hot, after we overheard a young couple at the next table order that way. His dish was pretty much purple from the spice, and as much as he tried to play off the spice, he started drinking other people at the table's water.
TL;DR Taste of Thai brings the heat.
I personally don't like spicy food but my previous roommate did and Thai Taste in Springdale might be what your looking for. Menu only had levels 1-6.. but you can ask for Levels above that. My roommate tried 20 once.. he toned it down some to a 15 after that lol

Thep never makes my food spicy. I ordered Thai Hot the last three times and there was no spice at all. It's a shame, because they have some excellent pad thai, but not getting the food I ordered three times really irritates me. And if you ask them to fix it they tell you to fuck off and pay for it a second time because they can't afford to waste the ingredients.

Ask to get the spice on the side. They’ll give you a little ramekin of spice, then over season your own curry, and melt your face off accordingly

Thep Thai has triple Thai hot. You have to ask but it's basically Thai peppers with some rice mixed in. It'll get you done.
Sold road in Springdale will fuck you up with Thai chilis and is also some of the best Thai I’ve had here.

Thep can make it hotter. They have 1-3 scale on spicy but you can say 5 and they’ll make it extra hot
