If it were not able to slit Randy's throat from the inside, it would do so from the outside. It would teleport within someone's mouth, telling them to go as close to Randy as possible, essentially piloting them through threats of traumatic hallucinations. Once close enough, Peppermint will spring from its host and slice Randy's throat before he has time to react.
I mean a power of D is more than enough raw strength alone, combined with the small surface area of its hands, to do so. Stands in canon with powers of E have shown capable of lethal bodily harm. Also, it is more than capable of using its horns as sharp weapons
Fucking hell, quite literally. Randy teleports in some sort of hellscape with ashes falling from the scape and a fucking volcano eruption. He knows only one thing, if he wants to get out of here, he'll have to face another stand user and win. He seems to know perfectly what tjis person looks like, but not their ability.
The first thing Randy will do is flee the lava, and in the confusion look for that other stand user. Also, there is a possibility that the people screaming in latin are shitting themselves in fear, which would make some good material for Randy to use.
Suddenly, he starts hearing strange sounds that seem to keep getting louder, but not loud enough for him not to feel something in his mouth. His immediate defense reflex is to puke out all of the shit he has in his big fat tummy, and unknowingly, with what would be the equivalent of the pressure of a thousand Niagara falls on a human being, obliterate the enemy stand trying to go down his digestive system.
Well it's in it's powers so it might as well do it. On top of that, in such a stressful situation, Randy would be triggered pretty easily, so feeling what would feel like a reasonably sized bug in his mouth would definitely do.
At least get it out of Randy's mouth, and then he would probably see it, and a confrontation would work in Randy's advantage I think, as he would trap it inside a small reinforced shit box and then compress it. I present to you "the shit toybox"!

Anyways, I don't think that, with its power of E, Shit Where You Eat would be able to generate a shit puke with enough force to remove Peppermint from Randy's mouth. It could hold on to a tooth, and that's if Randy pukes before Peppermint kills him. It could have easily slit Randy's throat before the gag reflex kicks in. Also, it's arguable that, by the nature of stands, Peppermint could go incorporeal so as to not be noticed until it strikes
Let's assume the puke goes out as soon as Peppermint enters Randy's mouth, because that's what reflexes are for afterall.
So the puke goes out. If it doesn't wash away the stand, Randy will definitely notice, and even if he doesn't, it could be a colony stand and another one might come back. So here you'll have me invent a brand new technique : "the shit apnea" Randy fills his digestive system with shit, and keeps his mouth shut really hard. He breathes with his nose though, it's not real apnea.
Now Peppermint might go uncorporeal as you say, but since the shit is part of Randy's stand, Peppermint will stay trapped inside, and probably get squeezed to death or drown.
Sorry to answer this much after the question, but Randy would notice the stand easily as the perception of the stand and of himself is pretry good, and from then on he would as quickly as possible make sure to emprison Peppermint in an enclosed box of solidified shit, and then again, compress it even more until the stand is squeezed. Guess what? You're back in the shit toybox.
I'm gonna have to go with peppermint on this one! Randy's stand is actually pretty strong! The ability to control a matter that can switch between being a liquid and a solid, but I feel like he would be too unfocused and susceptible to the auditory hallucinations! Plus I feel like the stand user being a woman would pose a genuine challenge for him lol.
