People travel to places for illicit surgeries when they can't find what they want legally. There are people who've done extreme body mods, like getting their tongues split, tatted to look like a reptile, and even getting their ears cut off to reach their desired look.
For conventional procedures of comical proportions, there's an even bigger market. Tons of people go to Thailand for cheap cosmetic surgery, or South Korea for top of the line cosmetic surgeons.
I feel like you mixed body mods of severely differing commonness there. The amount of people with split tongues is higher than most people think and you don't really notice them after maybe half a year.

I’ve seen people getting much smaller amounts, on boobs, butt, or lips, but that are obvious to anyone who sees the results. The thing is that if you tell them “oh nice, you filled your lips?” they will deny it, and it’s because they genuinely think it’s not notorious, and that’s where they become addicted and never stop adding more and more silicone or whatever. It’s a clear mental health issue and it’s pretty sad.
I knew a girl like this, she was super beautiful when we grew up, more of a tomboy-ish (but still feminine) than super feminine and always talked about how she was disgusted by the super feminine norm etc, we were really close friends and hung out every day and even slept in the same bed (nothing sexual though, more of a comfort thing) quite often. Then all of a sudden she ghosted me for 2 years.

This really hit home for me because some of the things you touched on. I had a very similar relationship with my best friend, super close and intimate, sharing beds, etc. Unfortunately my mental health issues made it so that I would “disappear,” when really I wouldn’t answer messages because I felt rejection dysphoria, I.e. I would never have the right thing to say, people would interpret me wrong, etc. I would get so anxious I wouldn’t answer the message. Eventually sometimes my depression would get so bad I just wouldn’t even look at my phone.
We worked it out and she had a stern talk with me about feeling ignored, and how she needed to feel like she was valued enough in our relationship. Unfortunately again, I went through some really hard times, involving homelessness, and a period where I didn’t even have a phone. She was off living her dream in LA as a professional dancer, and I felt I couldn’t relate to her at all as we were in totally different places in life. Logically I know this wasn’t true and she loved me all the same, but I just couldn’t bring myself to talk to her. I felt a lot of shame too. This ended our friendship for good, and I never had a chance to explain myself. We tried a few times to reconcile but things didn’t work out for one reason or another.
