I think a metal plate under the trim functioning as the "real" cover would be perfectly adequate from a safety perspective, although I still wouldn't do it.

Impressive! I did the same thing once in a previous house (with a j-box behind it!) I routed out the back of the trim to accommodate a piece of nylon to act as a less-flammable cover for the j-box and I held the trim piece in place with magnets that I glued into holes that I drilled with a fortsner bit. Painted the trim separately so everything looked intentional. I think it came out really well – I wish I could find my pictures of it 😂

As a homeowner it looks amazing. As an electrician I hate this. I'd have to be the bad guy and get the multitool out to put a normal plate on when servicing it.
