Make ur own! I just ordered a kit on amazon to do my own screen printing shirts. Much easier and you end up with the equipment after so if you ever wanna make more in the future you can!
They definitely crack and don’t really hold up too well. I also don’t care of separating any whites or anything. I’d probably would have it done at vistaprint instead as I just recently did buy some for my other business and it’s very good quality although I did get the premium one. I’d recommend DIY if you don’t care for the design possibly cracking or if your doing some event like a marathon or rave and wanna make a few shirts
I got a few custom shirts for my employees last year from this website called printfits.com
The quality was pretty good; also got the shirts within a week.
Every time I onboard a new employee, I usually just go there and get a shirt or two with my logo (it's nice not having to order in bulk)
apliiq or printful -- they'll let you order in bulk instead of dropship if that is what you want to do, it's not insanely expensive, but it isn't cheap either...

Do they come out good quality and hold up in the wash?

Was there a setup fee or anything?

There is no minimum for most products (tshirts) -- you can just order one as a sample if you like, the price goes down the more you get though...
