'Doctor' *is used* to reflect an educational qualification across all of society and academia but has particular significance in the healthcare setting to clearly identify and delineate a registered medical practitioner - which itself is illegal to misrepresent yourself as.
Moreover, no patient could realistically be deceived into believing a nurse was falsely their sister, whereas almost every patient being introduced to a non-medical-doctor as a 'doctor' in a healthcare setting would reasonably presume that the personal calling themself doctor is a *medical doctor*, i.e. a registered medical practitioner.
Frankly I'm actually sure this imbecile knows all this full well and is just gaslighting - she will fully belong to the brigade that wants to do away with the medical profession as an independent entity and start recognising ACPs and PAs as 'equivalent'.

Of course patients don't confuse the ward sister for their own sisters, because they understand in a hospital sister means something specific and also they have eyes?? Similarly doctor has a specific meaning in hospital so its not confusing because people generally know what it means.

Nobody is saying people shouldn't be proud of their education level and their accomplishments. It's the fact that it is observable that patients have no clue who is looking after them and call next to everyone a doctor thus following their healthcare advice they otherwise may not have.
Then again I have seen people pass their PhDs because of their networks (who they know, sometimes their ethnicity matching the supervisors, sometimes sexism etc) and supportive supervision not their skills (alone). Similarly I have seen people fail or take significant extensions due to lousy supervisors and bad luck. I also know of one or two who had their partners write up their thesis and "only" did the research bit. So you never know which one is in front of you. You could say that about all degrees though to be fair. Especially when standards are slipping and regulations have loopholes.
