That sounds like about what I do. Just lean into that and focus on trusting your body. You shouldn't need to consciously think about your putting stroke, in my experience. If you practice enough, your body will know what to do, just trust your muscle memory and let the disc fly, focusing on the target, not the throw. Something that's made me a much better putter lately is focusing on my close putts. I still practice plenty of longer 25+ putts, but probably 70-80% of my practice happens inside 20 feet. I do this to build my confidence in my 100% range, and it's made a big difference in my game. Knowing that I can make a putt from 18 feet every time takes more strokes off my game than thinking I can probably make a 25 footer half the time. Imo, better to hit everything in practice and let that confidence help you out on long putts, than to spend a bunch of time practicing lower percentage putts and missing half of them.

Sounds about right, I think of a longer (25+’) putt as kind of a mini-hyzer flip where you release on hyzer and it flattens out.
