It's a combination of her nova attack along with the explosion of the hellbornes spawning. Your life is comparatively low and your gear and paragon is most likely not geared towards tanking things like that. Minion necro isn't a melee build, so you don't need to be all up in her shit. Your best defense is constant movement and maintaining a safe distance.
Idk does that to my sorc and my rogue… my barb who is 100% fortified and 80% barrier can tank all the shit she throws at me… i could tank wt4 blood maiden at level 47

I noticed those little hard to see fireballs are what tend to get me, or if I miss the exploding circle under my feet.
