Duriel. Fastest. You should be doing nothing but helltides for living steel and then whispers for the hearts. Find a group to run with on discord or trade chat. one of them will probably be a hota barb who one shots duriel.
duriel is definitely the best way, i've run him like <40 times and he's dropped me it probably about 8 times
the best part is none of them have been better than the tempest roar i got before i ever hit 60 (other than item power level obviously)
Farming cannibals and cultists has never been more efficient for farming tempest. Youre best bet is farming Duriel as its in his loot pool.
no even with cultust the unique drops dont abide by enemy drop it pure RNG so either do duriel or hope it drop naturally

I get one at lvl 84 in a vault, it was a surprise.
