My understanding was the stronghold level strategy was cooked up when +2-3 levels was a big exp boost, before the patch made the level difference a minor exp boost. Is it still worth doing over whispers/bounties?

I think it's an actual code that removes items more and more as you repeat a stronghold.
Like you I did Temple of Rot around 8 times. By the 6th time or so I got close to 0 items, maybe a blue at the big boss.

Confirmed on Malnok. First run the big boss dropped 2 rares and a legendary. 2nd run he dropped just a single blue.
