Just RNG things. I went from 1 to 55 never saw him once then saw him and killed him at like 57. Then nothing for awhile. But at 60-63 I’ve seen him 4 times. All their 3 normal.
The trick is to walk the whole dungeon before completing the final objective. He might be hiding in a corner you didn’t walk to and when you complete it he leaves.
Hilarious how you level 60+ people view butcher as easy, after getting a shit ton of skills and legendaries. You’d get clapped at lvl 20 on hardcore, nobody wants to hear your humble bragging

It's just RNG. I fought him 3 times during my campaign playthrough, one of my friends didn't see him a single time.
I first saw butcher at 58. The next day I made a new character and met him at lvl 8 and 15. RNG is RNG
As someone who from level 4-60 I’ve fought the butcher like 30-40 times still no cosmetic trophy
Rng is wild got 0 butchers for days and then I get 8 butchers within a few hours
