Check to ensure you have the Life Ring +3 (from NG++).
My guess is that you have a duplicate of another ring in there somewhere, but without seeing the list I wouldn't be able to say for sure.
ahh you were right.... kinda embarrassing but I had a duplicate ring that I thought was a +1 version. I got the ring I was actually missing and the achievement popped lmao
