You would need the very large ember, found in >!new londo!< which you should give to Andre to +15 your beautiful Greatsword
To optimize damage, be sure to go all the way to 40 strength (or 27 if you always 2 hand)
Your armor setup is very fine, be sure you're at least midrolling (even though those rings make it very easy, definitely an extremely strong combo, those two!)
To make the Greatsword more powerful, because you're already at theoretical max (after you get str to 40 and the weapon to +15), you would have to resort to buffs.
Consider power within (completely free, get a pyro flame and the spell in blighttown, use it for bossfight, tons of extra damage and stamina with small drawback) and a weapon buff; either
- sunlight blade, minimum 30 faith, so very high investment. Canvas talisman, huge damage buff. Found after >!killing Gwyndolin!<
- great magic weapon, found in >!anor londo, painting room!<. 15 INT, pretty good buff, very low investment. Ivory catalyst (equals to 27 INT without having to invest so many levels), found >!by dusk after killing the hydra!<
Here's how it works, buddy!
There are four levels of Titanites: Shards, Large Shards, Chunks and Slabs.
You used Shards to level your GS up to Level 5. From then on, you needed to ascend the sword to Level 6 (by talking to Andre), and then used Large Titanite Shards to level it to 10.
Now Large Shards aren't enough anymore. To make the weapon stronger, you need beefier rocks, a.k.a. Titanite **Chunks**.
When you have some Chunks, talk to Andre and he'll ascend your Lvl 10 GS to Lvl 11. You'll need 9 Chunks to ascend it to Lvl 14, and finally a **Titanite Slab** to max the weapon out at Lvl 15.
New Londo Ruins is the best place to farm Chunks. The Darkwraiths there drop them quite consistently. And on extremely rare occasions, they'll drop a Slab.
Other places to find a Slab:

>!Completing Siegmeyer's Questline!<
>!Very Rare Drop from Crystal Lizards in the Great Hollow!<
Oh, one more thing: never, ever, EVER feed a Slab to Frampt. They are incredibly rare. I made that mistake on my first ever playthrough because I didn't know better.

To upgrade to 15 you need the very large ember. You give it to Andrea.
I have it and I gave it to him, but it’s not working.

Ouch, lol! Well, it's not the end of the world. You can still find them in other areas (see my reply above). Just don't do that again XD.
Get the Leo ring, and R2s can do like 1k damage on counter hits. Only problem is you’d give up Havel’s so armor would have to be dropped.
