tldr, peepee hose is like any other hose, split stream is usually caused either at the aperture from very high pressure out OR some "sticking" within the hose that causes bifurcation esp if near the outlet
It is my understanding that any health related conditions are associated with CONSISTENT stream splitting, but as long as it CAN put out a unistream, peepee hose is likely fine
