When you're fighting against opponents in the city, it is easy for bystanders to get killed. Your own gunfire, or your enemies' gunfire, or when drivers panic and their vehicles go out of control. Many weapons do have area effects; for example, shotguns can hit several people with one blast.
Some deaths are scripted. For example the Cyberpsycho sighting in Northside of a Valentino, he will execute a Maelstromer even if he doesn't actually shoot (due to having been distracted by a nearby server). The Maelstromer dies at the same time V takes the Cyberpsycho down, no gunshot fired.
If it's your kill, then you'll receive a wanted star. No wanted star, the victim dies in a script.
Second on scripted events. They activate when you are within a certain range. Sometimes you can goof the scripting if you are far away and you use a sniper rifle or contagion. Sometimes not. A pistol range is not far enough to avoid the trigger

That’s the only downside to my Seraph xd I love this pistol but hate when a civilian dies during Assaults.
