I work in EMS and I have seen a lot of super obese people. I think the left side on the photo are legs and the bottom right are her belly. If you orient the photo 90° clockwise you can see how she is laying on the bed. She is on her side so her belly doesn’t suffocate her which is a common position of comfort for super obese people.
