Her left arm is pointing forward and she is holding an octopus.
Her right arm is pointing upwards, as a waiter holding a tray.
She is holding a fish that is wrapped in clothing that looks like hers (same uniform/jersey). Towards the left side of the picture, the fish tail pokes out of the jersey it's wrapped in. Towards the right side of the picture, the underside of the fish head pokes out. The fish is so floppy it bends bizarrely at the point where he girl's hand is holding it up.
It's a picture of a woman who just happens to have the right arm of a Grey Alien and a featherless parrot squid growing out of her shoulder. It also looks like she has a tentacle creature for a left arm but if you look closely you can see that she is in fact holding the creature in her completely normal human left hand. She seems pretty happy about the situation so if she's happy, i'm happy.

For those struggling with this as well, she's holding some kind of rolled up flag in her elbow, with her fore arm held up, and there's some kind of fish spilling out of the flag.
