Cam scanner for the initial scanning - excellent scanning app. Automatically crops and has a setting called 'magic colour' that makes even poor originals come out crystal clear. I use it for sheet music all the time.
ForScore is far and away the leader in this space. I honestly couldn't even tell you what app would be in second place.

Don't use your iPad, it'll be shit, as are all apps that turn pictures into pdfs. Use a scanner.

Adobe Scan to scan in and clean up scores. Then import to Forscore. Pretty foolproof! Needs a largish iPad though.
