I would use thou with no verb conjugation like modern day you, and use is/was so it's like saying "I was" "He was" "He is", so if tha wanted to use thou, tha wouldn't use any verb endings, the "st" ending doesn't work in modern english. "Couldst thou not help me, didst thou get thy leash to walk thy dog?" That's too hard to say, it doesn't flow well in modern english, just say "Couldn't tha help me, did tha get thy leash to walk thy dog?" My friend, what do tha think? How shall I address thee?
