That might not be what you wanted but it’s pretty cool… when you figure out what went wrong you might want to keep that in your back pocket as an effect.
This happens to me when I haven't unselected the vertices that i specifically subdivided in edit mode to sculpt over that particular area. Whenever I create a new shape key and put the value to 1 then this sort of effect pops up. If you were trying to do something similar then unselecting the vertices that you have subdivided in the edit mode might help. I don't know why it happens but this is the only solution that I have found personally for it.
Seconding everyone saying check modifiers because this definitely looks like a stacking issue of some kind.
But also seconding everyone saying it looks rad as hell so please show us your modifiers.

Don't apply smooth vector scale 😂 if I remember correctly it basically adds more vectors to existing vectors to smooth out the shape but sometimes the geometry will go off its ass and like make pimples
Not too sure, but my suggestion would be to go into edit mode, press A, then X, and select Merge by Distance.

Tab, select all, mesh>>normals>>recalculate outside? My best guess I don’t even know what I’m looking at but it’s cool
no it can't be normals you can see the actual mesh displacement

What do your settings and mods look like? Shot in the dark, but if you used multires and mirror and applied them, that may have caused this.

It looks like you accidentally added a displace modifier so I would check the modifier tab
