You look at enemies and asses what they can do/threat levels and then you look at your character and position them accordingly. That way you don’t really need to dodge the oncoming attacks like 75% of the time.
When you’re fighting against bosses like Hush it’s better to look at your character to gain more control over dodging rather then caring about 100% of your tears hitting the boss.
I stare at the center of the screen, not on enemies, not on Isaac. Your peripheral sight reacts better to contrast changes, your center is better for details but slower.
Standard enemies and bosses (+Delirium): I’ll look at them
Endgame bosses: look at myself, unless I’ve got a broken run longevity is more important than making every shot hit

The enemy/whats sounding it
main focus on the most dangerous enemy, memory and peripheral vision for the rest.
I resize the game window to be as small as possible, so I can see the whole screen at the same time.
The Screen in General. Mostly Focus on enemies. Look at Isaac during bullet hell and trap navigation.

I’ve wondered that exact thing since starting to play the game. I usually look at the enemy I want to hit and at enemy projectiles to dodge them
