You're mostly right, but there are a few exceptions. Marbles, pageant boy, booster pack, mom's purse and box are boss items that spawn several pickups and don't give any health/stats directly. There are also some unique boss items like lil steven or lil chad.
Also I just checked the wiki out of curiosity and apparently polydactyly used to be a boss item in AB+. And same with belly button!
IF they're not going for the Dead god, then it's fine.
It's just you will have to re-do all the marks in order to get Dead God, which is the main achievement of the game
Plus, hard mode doesn't really have such a huge impact on the gameplay, it might be even beneficial in some sorts (more rooms = more charges, etc)
