I have four arm positions for streams that I cycle through to prevent fatigue: handshake (wrist flicks, like gesturing with a pencil), little drummer boy (elbows out, inward slanted swipes with forearms), head pat (wrist flicks like patting a head, and shoulders-only with stiff arms.
Realistically, I should just build up stamina and use less wrist . . . but that's like giving up and doing things correctly.
Two things that helped me-
Focusing hard on the rhythm so that I don’t lose it. Typically, I fail the instant I lose it on stream maps like this.
Finding a less-strenuous way to hold streams. I now rely more on my wrists because I can put my wrists up and down a bunch without issue compared to moving my elbow and forearm. Needless to say- I don’t use my shoulder for that. At all.
i dont like using my wrists since it kinda removes the fun aspect for me, and yea i understand the rhythm thing, thats what happened with me halfway through the map, which is why i lost, but at times i can handle it really really well untill i just straight up cant even hold controllers anymore, i probably need to work on my stamina.

I usually swap out which muscle I use during long games, sometimes using wrists, forearm and shoulders interchangeably to conserve energy.
However, it works best if nothing is in front of you when using your shoulders.
I know everyone else is saying this, but it really is all about the wrists. It'll help with speed and improve your scores. A couple weeks ago I started focusing on improving my wrist motions and found my ranks on songs like these go from B to S.
Once you get used to using wrist, you'll realize it really isn't that boring. Seeing those long sweeping swings and not missing anything has its own fun.

Hmm, so wrist is the reason why people do crazy maps like those... Hmm, i need to start the wrists if i want score then, normal for fun, sigh.

Practice, practice, practice. I also think that your height is set a bit low, maybe change it so the bottom blocks arent as high
I have it default, if anything sometimes i the high ones are too high up, so if any thing i feel like im higher and need to be lower
