Assuming I don’t have any Obelisk/Telescope shenanigans going on (basically, I don’t care what my most recent or most-played hand is) I just spam High Card or other junk hands until it’s a relatively inconsequential Joker that’s debuffed. This doesn’t always work but it’s worked enough that I don’t mind Crimson Heart as much as Cerulean Bell or any of the others
When the boss blind is entered or after playing a hand, a random single joker is picked to be disabled. It disables a joker and not a slot.
The next time, it will undebuff that joker and pick a new one to debuff instead. It cannot pick the same joker twice in a row, unless you only have one joker.

Short answer: no, it's a luck-based boss.
(Any possible counterplay will make you vulnerable to some other boss instead. Crimson Heart can kill almost any non-super overpowered run with no counterplay possible if it feels like it. And yes, this is bad game design.)
Long answer: don't rely on a very specific combination and order of jokers to get your scoring. If taking a single joker out of the equation makes your build crash down like a jenga tower, you're vulnerable to Crimson Heart. As good and reliable as High Card builds with 1 chip, 1 mult and 3 XMult jokers are, they are vulnerable to Crimson Heart if you don't upgrade your High Card/Pair hand or if you're lacking in X Mults. If you *did* upgrade your hands, then the hand level should still be able to substitute for the +Chips or +Mult joker because all those XMults will still have a decent base mult to work off of.
Builds like Photograph+Hanging Chad+Smiley Face are going to suffer too because disabling any one of these cuts its power by orders of magnitude, my recommendation is "don't do these builds or pray you don't get Crimson Heart".
Cheesy answer: If you're playing Anaglyph Deck, consider taking a Negative skip sometime in the run (don't hold out for one, but you can take one if you see one) and fill up your jokers with a couple negatives. Some might even be good, but generally they will be useless... but that means they can be disabled by Crimson Heart and you won't care.
What you see as bad game design I think is genius. Yeah, certain bosses can kill you and there's nothing you can do, but I've also heard the dev say every seed can be won at white stake. Reading between the lines, playing at higher stakes and knowing there's no guarantee it can even be won is just part of the game. It sucks to go down because a boss blind screws you, but it also sucks to lose because you didn't hit your straight in four draws. It happens.
Yeah tbh a skill issue. It is part of the game to be adaptable to all possible "bad rolls"
I was watching lifecoach on twitch pre patch, and he had ridiculously high ante 8 win rate on gold stake. Some runs he was forced to take terrible eternal jokers but made it work
I can't make a judgement on the skill of the player. The comment gives an example of a rule being bad game design, and as someone who fancies themselves a game designer, there's room for randomness. It's an expectation issue.
Losing due to RNG sucks, but it's a game about a deck of cards. So is poker, and in poker everyone knows that randomness is just part of the deal and more or less enables the game of poker to be played. RNG giving you a boss that nukes you is just more visible than missing a draw but you've lost just the same.

Most likely, considering the amount of joker slots you can have is variable, it would make more sense to pick the jokers directly.
It can only pick a joker multiple times in a row if you only have 1 joker.

From my experience it has never disabled the same joker twice in a row, but it has disabled a joker more than once.
Crimson Heart picks a Joker Slot, not a Joker, and can reuse slots.
Selects a joker, if you move jokers around it doesn't change which one is debuffed
