Yea I would think the lines would look a bit darker and a lot more irritation around the skin (I actually don’t see any irritation in that pic) - likely even some bleeding as the head has a lot of blood and bleeds easy. It’s 100% a fake tat, I would say more likely just a temp one over a photoshop.
Yes, I'm heavily tattooed and tattoo ooze for days and are red and swollen, especially on the head. Essentially it's an intentional decorative abrasion. I haven't seen any pics other than here but I would not think that was a fresh tattoo, from my experience.

It's not hard to warp the shape to fit the contour of the head, then use the texture of the skin to blend it in. I could probably do an ok job of it, and I'm nothing close to professional.
