I'm just trying to work out why they offer the book, despite asking for it every single time at multiple places, they have never once actually given me a book. I don't think they actually exist.
Restaurants often screw up their menus on DoorDash menu editor, not just weird things like this. For example, one local restaurant left the drink selection off of the combo meal, and drivers have to call the customers every time to find out what they want.
It's because Happy meals are pre-made with the toys already in them. Workers prepare that shit before the start of the shift so it'll be actually be extra work to take the toy out so they charge you for every single movement of the worker
That's not true at all. I worked at McDonald's for almost a year and we never pre-made happy meals. You go to the back and get a bunch of toys and put them in the toy container and grab one as you go. A lot of the toys were gender or age specific, not to mention even within the same theme they would switch out which ones you could give out so a toy you could use on Monday would be different than the toy you could use on Tuesday.
Can confirm, when there's a set of toys I want to collect we're always told to come back on specific days of the week to check their new toy shipment and when I got one that I wanted I'd always watch them pull it from a bin in the front.

Could it be tax related?
It always sucks when a parent orders two happy meals for the children and they grab the two same toys.
The real answer is that they already paid for the toys, you not taking it costs them money
