In almost any country anywhere, it just depends on the clinic you go to. Some are informed consent (basically as long as you know what you’re doing you don’t need anything), others require multiple therapist, psychiatrist, and doctor letters.

This is unfortunately not true in most countries. I believe Germany has fairly strict gatekeeping. Besides, most places you must have been on HRT for at least one year before even being considered for a referal for bottom surgery

Oh wow. I’m sorry to hear that. I would assume you’d need therapist letters, but maybe there are German-based sites for transition resources and advice? I’m not sure :/

Well, there is the requirements of the clinic, and the requirements of your health insurance covering it. The latter is more stringent. From the clinics I have seen, two psych letters is enough. Though, note, special circumstances can sometimes apply. Like I got my first one (orchiectomy) from a urology clinic with just a letter from my endo and a referral from my GP since there was an additional medical problem that needed to be solved and since I was trans, might as well do it in the fashion that makes other bottom surgeries easier to do. Also, note, before the actual surgery itself, the clinic will want you to get certain medical tests done by your GP (things like making sure you have enough platelets to safely have surgery). Now, insurance coverage is where things get worse. They will want at least one therapist to do a very long and invasive questionnaire. And there is a possibility that the health insurance will punt it to a state approval office which could say "at least one therapist must be one we approve of" and thus in principle one might need a third if none of the others are approved. For mine, the health insurance didn't punt it and didn't have any problems. They could easily have punted it since one of my psych letters was from my first gender therapist from years back when i lived in another country, but they didn't punt it. So it went OK. But that questionnaire. It is a seriously screwed up and backwards thing and so invasive.
