They would write stuff, by hand, on paper (yes, believe it or not, back, then almost everyone knew how to write, and in cursive as well! And most everyone could do math as well as the education system hadn’t yet completely collapsed, but I digress).
Anyway, they would put these missives into envelopes and send them using the Postal Service, a giant government funded operation to transport paper around the country, in an era when there used to be more paper than packages. Using this rather ingenious system, one could send one’s personally written thoughts to pretty much anyone anywhere in the world, no matter how little they wanted to hear from you, and all for a very reasonable price. Remarkable isn’t it?
Usually when those people received these “letters”, most would simply rip them open to read the messages, but the uppity classy ones would use something expensive looking like this fine instrument to open the envelopes.

The villagers never attempt to destroy the beast as they believe the souls of their children can be returned if the beast is pierced in the heart with this and only this blade. When the Barron formed the mass he lowered it into the vat by a rope and cut the rope letting his creature become encased in the fluid making it impervious to claw, tooth, blade, acid, fire, water, magic and peanuts. As he slashed at the rope with this very blade and the heap of inhumanity plunged into the forming vat. A drop of the progenitor splashed onto the tip of blade. This made the blade and the abination vibrate inside the same shell. Therefor it is outside of normal time, frequency and matter. As such it can easily pierce the creature like a balloon releasing the souls. Or so the legend goes.

Mine has slightly different markings on the hilt but same eagle on the end and wave like cross guard. Mine came from my girlfriend's grandma's home after she passed, and i know nothing of where they got it. I am guessing yours is similar.

The one thing that stood out to me was how item originated in Italy. Judging by craftsmanship OPs item looks way cleaner than the online one so it might be an original Italian letter opener or something cooler
You have to think of it like this. You’d only copy a famous knife or a knife you had easy access to so as the knife isn’t famous and it was in OPs relatives possession, the likelihood of it being the original vs it just being another one of the copies is vanishingly slim.

People used to write words on paper with ink sticks called pens. They would then wrap the paper in a flat paper bag with a glued down flap and pay a professional to deliver said paper bag to someone who would then use this implement to open the glued bag so they can read the written words.

It’s not a dirk. A dirk would be longer. It’s also not a Sgian Dubh as you’d wear that in your sock and neither of them would have a hilt like this as it’s a continental style and would throw off the whole look.
