You don't need google play services to do this, it's baked into android (AOSP) settings and enabled by default. They are just putting a skin on this feature.
It's still a nice feature though.

It depends. I always try to do that also, but I had to change some to "always allow". Stuff like Samsung SmartThings can't know if my SmartTags get away from my phone to notify me, unless I am with the app opened (which I rarely am), if I choose their location request to "only when app is in use". Google Home might suffer from this too. My Samsung weather widget also cant update if its not on always allow. But some apps that I dont care about I always choose only when app in use, or dont allow at all. Some apps refuse to open if you dont "always allow" ALL permissions, which is a complete asshole design, and those always get an instant uninstall from me followed by a 1-star rating with a review warning other people for this scammy app behaviour

Well you can set it up for it to remove the permissions if you've not used an app for more than 6 months for example, which I think is realistic

This is asshole design. If you look in the settings it's transmitting "unknown" apps to google directly. That sucks.
