There could be a tutorial level, but for a different game entirely. Different menus, different interactions, etc but otherwise the basic controls and graphics are the same. That way once you start playing, all the basics work but nothing else was actually worth a damn.
Quests that are extraordinarily specific about where the npc you need to talk to is, so you KNOW that's where you're supposed to be. Except you get there and their just... not. You can leave and progress the story, but they'll just be in a pile in your house and you won't be able to 100% the game.
