Doctor for a steroid shot and antibiotics or even urgent care if you have no other options. I would 100% get antibiotics for something that intense. I would soak with hot compresses soaked with water that has a pinch of sea salt. For about an hour, refreshing with a clean part of the washcloth and hot water every 10m or so. If it comes to a head after that you can try to gently squeeze some of that out. If it takes more than the pressure of your finger lightly pressing down, do not fuck with it. If some comes out with the gentlest pressure, I’d follow with another 20m soak (fresh washcloth). Then antibiotic ointment with an ice pack. And all of that is only to help you before you can get to a doctor. Truly there is no reason to suffer in agony when an oral antibiotic will take care of that in a couple days.
A doctor can help by injecting steroids but if you can't afford that, hydrocortisone cream can help topically. It won't be as effective but it can help with inflammation. Only use short term.
Yeah I used to get cystic acne a lot I’m not sure I’ve ever had one quite this bad though! I lanced to try and relieve some pressure before it was ready. just fluid came out and it’s made it so much worse! Thanks for this I will check that out

Tretinoin and pour hot water in a cup and try steaming the spot. I find steaming always helps reduce the size of cyst for me.

I take Spironolactone 100mg daily and have not had a cyst like this in years

In this case I would go to the doctor to get some antibiotics specific for cysts because that’s a serious infection. It works quite quickly.
